She should call the US embassy in the Phillipines. It might take a little bit of persistence depending on how helpful the staff is at that particular embassy, but they'll be able to give her the step-by-step on what she needs to get a work visa.

Only employers can apply for employment visas, and she does not have the necessary qualifications to meet visa requirements. She needs at least a bachelor's degree plus post-baccalaureate experience. Also, she needs a state license as a Certified Nurses Aide in many states now to perform in-home health care services, and she cannot sit the board exams unless she is a citizen or legal permanent resident. For nannies, there is the Au Pair Program. She would have to go through an authorized agency in the Philippines for screening and placement. She would be allowed into the US for a maximum of 1 year if she passed all of her screening tests and was hired in the US. Afraid I cannot recommend an au pair agency in Philippines -- the only ones I'm familiar with are in England and Sweden.
